Evert Health is a visiting physician service that sends board-certified doctors and advanced practice providers to homes, group homes, and assisted livings for patients that are home-bound. We practice primary care with a palliative care heart. We are honored to care for your loved one in their own home.Our ability to visit patients in their homes, group homes, or assisted livings throughout much of Oakland and Macomb counties makes us an excellent choice for in-home care for those that need it.
