Sildenafil works by inhibiting the PDE5 enzyme, which increases levels of nitric oxide–mediated cGMP. This relaxes smooth muscles in blood vessels, improving blood flow. While this is helpful for erections, it also causes widening (vasodilation) of blood vessels in the skin, especially in the face, neck, and chest. This increased blood flow near the skin surface is what leads to the warm, red “flushing” sensation.

It happens only “sometimes” because the intensity varies from person to person and even from dose to dose. Factors that increase flushing include higher sensitivity to vasodilation, alcohol consumption, anxiety, hot environments, or taking the tablet on an empty stomach. Some individuals naturally metabolize sildenafil differently, so their blood levels peak more strongly or quickly.

Flushing is usually mild and temporary, often fading within a few hours as the drug level decreases. It is generally not dangerous, but if it is severe or accompanied by dizziness, chest pain, or fainting, medical attention is needed.
